All three services are delivered by Ann personally — in the clinic, via telehealth, and in partnered assisted living facilities.

Psychiatric Care & Medication Management

Full psychiatric evaluation, diagnosis, and ongoing medication management for adults — delivered by a board-certified Psychiatric Mental Health Nurse Practitioner who also happens to know your medical history.

  • Grief & life transitions
  • Cognitive changes in older adults

What to expect

Initial evaluations are 60 minutes. Follow-ups are 30 minutes and typically monthly at first, then spaced out as treatment stabilizes. You will see Ann every visit — not a rotating panel.

Family Primary Care

Preventive care, chronic disease management, and everyday medical needs for adults. Ann's FNP training means she can address the physical alongside the psychiatric — because they are almost never actually separate.

What's included

  • Annual wellness exams and preventive screening
  • Chronic disease management (hypertension, diabetes, thyroid, etc.)
  • Medication reconciliation — especially for patients on 5+ meds
  • Acute visits for common illnesses
  • Referrals to specialists when needed

Not currently accepting pediatric patients.

ALF & In-Home Telehealth

A dedicated telehealth practice for assisted living communities and homebound adults. Residents receive consistent psychiatric and medical follow-up without leaving their room.

How it works

  • Ann visits your facility (virtually) on a regular schedule
  • Facility staff assist with the technology — residents don't need to
  • Notes and orders back within 24 hours
  • Designated family contacts receive an update after each visit
